Agape Family Home is a leading provider of comprehensive care including skilled nursing, home and community habilitation, behavior support services, and other group home services in a safe and healthy environment.​

Developing a customized plan of care based on your loved ones needs or wants ensures that your loved one is receiving the care they need when the need it. Our services are of the highest quality and are specifically structured around the specific needs and goals of each individual client. Our goal is to offer our clients the very best personalized care, allowing them to live safely and comfortably in their home.  Sometimes life’s natural events - aging, illness or disability can make living independently at home more difficult. Our services are available to you 24 hours a day, 7 days a week, wherever home may be. We specialize in hourly, full-time, and live-in care.

Agape Family Home is licensed by the Pennsylvania Department of Human Services/Office of Developmental Programs and is qualified to provide the following services:

1. Community Living arrangement for persons with Intellectual Disability (ID) and Adults with Autism Spectrum Disorder

Agape Family Home provides a safe and healthy apartment homes and single-family homes for 1-4 family members with mild to severe ID and ASD. Our community homes are staffed 24 hours with trained and caring Direct Support Professionals who support Individuals acquire independence skills, engage in community and everyday life activates of their choice.​

2. Home & Community Habilitation

Agape Family Home supports Individuals maximize adaptive skills, self-preservation, healthy choices etc.  for successful independent living and community participation. We provide this service in the Individual’s home or in Agape community homes not subject to licensing requirement.

3. In Home and Community Support

Agape Family Home assist Individuals who are living independently, with family or friends with Activities of Daily living, transportation to and from the community, doctors’ appointments, recreation etc.

4. Companionship

Because of some severe health problems or old age, some people cannot or do not frequently leave their residences. Agape Family Home caregivers are trained and equipped to provide needed and appropriate companionship services which include having a conversation, watching TV   together, reading aloud, or running light errands and doing light housekeeping.

5. Skilled Nursing (RN and LPN)

Agape Family Home Nurses provide skilled medical care including but not limited to wound care, monitoring of vital signs, tube feeding, managing chronic conditions.

6. Out of Home / In Home Respite

Families and caregivers sometimes need to take vacation or are too sick to care for a loved one, Agape Family Home provides that temporary relief and support as needed. Our trained Direct Support Professionals are standing by to provide quality care for your love ones in the comfort of their homes or in one of Agape Family Home’s respite homes for up to 30 days. 

 

7. Behavior Support Services

Agape Family Home’s trained and qualified Board-Certified Behavior specialist are equipped to conduct functional behavior assessment and develop evidence-based interventions to help Individuals reduce/ eliminate problem behaviors and increase socially appropriate/adaptive behaviors. Our Behavior Support Specialists also train Direct Support Staff and families to implement behavior intervention strategies.
